Eventually, to keep costs down, a lot of insurance will have to go away (keeping hospital administration costs lower by removing bureaucracy) and the enormous leverage of Medicare must be used to keep costs in check. There's a lot of cripples who are nervous about M4A since the Sanders bill completely ignores stuff currently covered by Medicaid waivers like personal care (especially in "good" States like NY and CA).
